Bio-revitalization without illusions: what works and what doesn't
A couple of years ago, I thought that bio-revitalization was an overrated "ritual" that patients love for the feeling of the procedure rather than for the effect. Today, I've revised my position. Not because the products have become magical, but because I began to look at them differently.
The key is not hyaluronic acid, but fibroblasts
When we inject HA, especially non-stabilized, it’s not about "hydrating the skin" — that’s marketing fairy tales. It’s about stimulating the cell cycle, about the fibroblast's reaction to local micro-damage and the presence of substrate.
If nothing happens after the injections — most likely, you either injected too little, or not in the right place, or the patient lacks the resource for regeneration. Yes, that happens too. Sometimes the skin is "more tired" than we think.
Schemes that work for me
Conditions:
Patients aged 35–50, chronostasis + photodamage
Non-smokers, no systemic diseases
No course of peels/lasers in the last 4 weeks
Scheme:
Preparation — 1.8–2.2% non-stabilized HA (I love Teosyal Meso Expert, RRS HA Injectable)
Areas — face, neck, sometimes décolletage
Technique — papules 0.03 ml, 1 cm step
Course — 3 procedures with a 2-week interval
Support — after 3 months, if I see a decrease in tone
And most importantly — I explain to the patient that this is not a "magic button" for 3 days. This is a reboot of the skin, and it requires time.
Problems that are usually left unsaid
Swelling after injections in the lower third of the face — often encountered. Especially in patients with significant subcutaneous fat or lymphostasis.
Hematomas — yes, even with perfect technique. I stopped worrying if they occur, the main thing is to explain this in advance.
Pseudo-effect the next day — due to swelling, the skin appears "full," but that’s not the result. Explain this.
Is it worth adding amino acids and peptides?
Sometimes — yes, but only if you know exactly what you are injecting and why. "Everything in one" products are not always better. If the skin is in a state of chronic inflammation, an abundance of actives can provoke the opposite effect.
Bio-revitalization works. But it only works in the hands of a doctor who understands why they take the needle in their hands, what kind of skin, in what context. And yes — sometimes refusing the procedure will be the best decision rather than pushing a course "just because it's time."
